1. Key takeaways from the month
Between Sep 24 and Oct 23, 2026, the GLP-1 market moved through safety data, one European recommendation and enforcement, while sales and prices changed little. Five conclusions explain the month; each is backed in the chapter named in brackets, with every fact judged against Oct 24, 2026.
1. The month brought a heart-safety result, not a new weight-loss record. In ACHIEVE-4, Lilly's oral Foundayo (orforglipron) met non-inferiority to insulin glargine on cardiovascular events, with a MACE-4 hazard ratio of 0.84 (95% CI 0.59-1.20) in 2,749 adults with type 2 diabetes, and superiority was not shown. Novo's CagriSema beat tirzepatide 5 mg, 12.4% against 9.1%, but at a low comparator dose; at the top dose CagriSema missed non-inferiority in REDEFINE 4. Retatrutide's 28.3% at 80 weeks remains the highest figure. [Chapter 2]
2. The only new decision was a recommendation, and access runs through national launches. On Oct 15 the EMA's CHMP backed Foundayo for obesity and type 2 diabetes; the European Commission decision is pending, and about Dec 21 is our estimate from a 67-day norm. Novo said on Sep 30 that its Wegovy pill is launched in the US, UAE, UK and Germany, while a Sept 2026 tracker found 2 of 16 European countries on the market and 13 approved but not launched. CagriSema's US decision is expected in Q4 2026. [Chapter 3]
3. Lilly leads, but Q3 results arrive after the window. Lilly reports on Oct 29 and Novo on Nov 4. In Q2 Mounjaro and Zepbound sold $9.9B and $4.9B, about 1.9 times Novo's $7.8B injectable semaglutide, and Lilly held about 61% of US prescriptions. Foundayo made $98M in Q2. [Chapter 4]
4. Supply news was enforcement, not scarcity. Apothecary Pharma recalled 22 tirzepatide lots plus one NAD+ lot for lack of sterility assurance, classed Class II on Oct 8, and FDA's Sept 18 letter to Empower called its products "essentially copies". Neither drug appears on FDA's shortage list, and the plant figures, such as Lilly's more than $20B for injectables, are cumulative commitments with no in-window announcement. [Chapter 5]
5. The price now depends on the channel. Medicare's Bridge, in effect since Jul 1, has a $50 copay against a $245 net price, and cash buyers pay $349 for the Wegovy pen or $149 for the starter Wegovy pill. Commercial coverage is retreating: Zepbound is uncovered for 60% of commercial lives, and at least 10% of covering employer plans may drop coverage in 2027. [Chapter 6]

2. Clinical data
Chapter 1 summarises the month; this chapter gives the trial data behind it. Separate trials are never head-to-head, whatever the gap.

| Meals or app after GLP-1 stop (pilot, n=39) | regain ~3.6% vs ~11% | Oct 7, 2026 | Peer-reviewed pilot | News-Medical |
No new efficacy record arrived. The month brought a cardiovascular safety result for an oral pill, a CagriSema win over a low tirzepatide dose, and first randomized data on limiting regain. Retatrutide's 28.3% at 80 weeks (TRIUMPH-1, May 21) remains the highest figure.
2.2 Novo Nordisk: CagriSema wins at low dose, not at the top
Novo reported REIMAGINE 5, a Phase 3 trial in adults with type 2 diabetes. CagriSema 1.0/1.0 mg gave an estimated 12.4% weight loss at 60 weeks against 9.1% for tirzepatide 5 mg, a gap of 12.4 - 9.1 = 3.3 points. HbA1c fell 1.71% against 1.67%, meeting non-inferiority. This is a company announcement with no peer-reviewed paper or safety table in the material.
The comparator was not tirzepatide's top dose, and the only top-dose test went the other way. In REDEFINE 4 (Feb 2026; 809 adults; 84 weeks) CagriSema 2.4/2.4 mg missed non-inferiority to tirzepatide 15 mg: 23% against 25.5% on treatment, according to pepglp1. In REDEFINE 9 the 1/1 mg dose gave an estimated 21% at 68 weeks against 2.0% for placebo, a 19-point gap (21 - 2.0).
On Oct 20, 2026, at the Menopause Society meeting, Novo presented a post hoc analysis of REDEFINE 1: women (n=2,309) lost about 25% at 68 weeks against 3% on placebo. By stage the figures were 26.7% (premenopausal), 24.8% and 23.1% (postmenopausal) against placebo at 2.1%, 1.7% and 2.6%, or 24.6, 23.1 and 20.5 points placebo-adjusted. The Motley Fool notes these assume everyone took the drug as directed. Medical Daily reports digestive side effects in 79.6%.
CagriSema's best numbers come from favourable subgroups or comparators. Its US decision date is in Chapter 3.
2.3 Lilly: a cardiovascular safety result and two combinations
ACHIEVE-4 is the orforglipron (Foundayo) MACE trial: Phase 3, open-label, against titrated insulin glargine, in 2,749 adults with type 2 diabetes and raised cardiovascular risk. Lilly released the detail on Sept 30, 2026, as it was presented at EASD and published in The Lancet.
MACE-4 (cardiovascular death, heart attack, stroke, unstable angina admission) had a hazard ratio of 0.84 (95% CI 0.59-1.20), with event rates of 4.2% against 5.0%. That met the non-inferiority bar (upper limit below 1.8). MACE-3 was 0.77 (p=0.181), so superiority was not shown. Lilly's 53% and 57% lower cardiovascular and all-cause death (HR 0.47, 0.43) were not controlled for type 1 error. At 52 weeks weight fell 8.8% against a 1.7% rise on Lilly's efficacy estimand, and -7.79% against +1.12% on the treatment-regimen estimand (Medscape); we quote both.
On safety, gastrointestinal events were 62.1% against 14.2%, and adverse-event discontinuation was 15.4% against 5.4% over the full trial. Lilly gives 10.6% for the first 52 weeks, so the windows differ. Level 2 or 3 hypoglycaemia was 6.8% against 19.2%. Investigator Klara Klein has consulted for Lilly; the independent discussant was Gian Paolo Fadini. Sources disagree on the trial's dose ceiling (17.2 mg; 36 mg capsule) and Lilly's text omits it, so we state none for the trial; the EMA's top tablet strength is 17.2 mg (Chapter 3). ATTAIN-OUTCOMES, powered for benefit, completes in 2031. A separate Oct 1 Lilly release gave modelled, not observed, ATTAIN-1 estimates.
Eloralintide plus tirzepatide. In a 48-week Phase 2 trial in 367 adults with type 2 diabetes, the top combination gave 23.3% against 14.8% for tirzepatide 15 mg and 3.0% for placebo, an 8.5-point gap (23.3 - 14.8). Side-effect discontinuation was 10.8-27.0% on the combinations against 2.9% on tirzepatide. Leerink's $23 billion 2035 sales view is an estimate.
Retatrutide reached EASD with a completed Phase 3 programme: 20.8% at 12 mg after 80 weeks in obesity with type 2 diabetes. Lilly plans a US filing in Q1 2027, according to glp3.wiki.
2.4 Regain, switching and the wider field
Fractyl's REMAIN-1 (Oct 16, 2026) tests Revita, a duodenal ablation device, in a sham-controlled trial after GLP-1 therapy stopped. Regain was 5.1% against 8.2% (p=0.0023), a (8.2 - 5.1) / 8.2 = 38% relative reduction. The release lists arms of 176 and 74 against N=308 without explaining the gap (176 + 74 = 250). The 53% reduction at 20 cm or more is post hoc. The second co-primary endpoint reads out at 12 months, planned for Q1 2027.
The only randomized behavioural data is a 39-person pilot in Obesity Science & Practice (Oct 7, 2026): meals (n=13), the Noom app (n=12) or usual care (n=14) for four months. Model-estimated regain was about 3.6% in both active arms against about 11% for usual care, roughly 7 points apart. Weight was exploratory, prior weight loss differed by arm, and follow-up was short.
Novo and Ro presented non-randomized real-world data: patients who switched to the Wegovy pill and stayed three more months lost an extra 4.1%.
Amycretin, now zenagamtide, has no Phase 3 efficacy data. Its trial AMAZE 7 was registered to start Sept 22, and AMBIENCE on Oct 26, 2026.
3. Approvals and launches
Chapter 2 covered what the trials showed; this chapter covers what regulators and launch teams did with them between Sep 24 and Oct 23, 2026, and the one new decision in the window was a recommendation, not an approval.
3.1 Decision and launch tracker (as of Oct 24, 2026)
| Item | Key figure | Date | Status | Source |
|---|---|---|---|---|

3.2 Foundayo: from CHMP opinion to the EC decision
The only new GLP-1 obesity decision in the window was a CHMP opinion. Foundayo is recommended in the EU, but no tablet can be sold there until the European Commission (EC) decides.
On Oct 15, 2026 the EMA's CHMP recommended a marketing authorisation for Foundayo (Eli Lilly Nederland B.V.), an oral, non-peptide GLP-1 receptor agonist. EMA's own release is the primary source; Syenza and PharmExec date it Oct 16. The indication covers two uses:
- Weight management: adults with obesity or overweight who have at least one weight-related health problem, alongside diet and physical activity.
- Type 2 diabetes: adults whose blood glucose is not sufficiently controlled, alone when metformin is not appropriate or with other diabetes medicines.
Foundayo would come in six tablet strengths, 0.8 mg to 17.2 mg. The opinion rests on two weight studies in 4,740 adults and four diabetes studies in 3,765 adults, compared with placebo, dapagliflozin or oral semaglutide (results are in Chapter 2). The October CHMP meeting recommended six new medicines with no negative opinions on new medicines, and gave a positive extension opinion to Novo's Kyinsu (insulin icodec / semaglutide), whose indication the page does not state.
The timing of the next step is a source conflict. According to Syenza, the EC normally decides within 67 days; PharmExec says the EC usually follows within a few weeks. We use 67 days as the outer bound. Oct 15 + 67 days = Dec 21, 2026 (16 + 30 + 21), and 58 days remain from Oct 24. That date is an estimate, not an EC statement. After the EC decision, price and reimbursement are decided member state by member state.
In the US, Foundayo was approved on Apr 1, 2026, 50 days after filing and 294 days ahead of its Jan 20, 2027 PDUFA date, under the FDA's national priority voucher programme (according to NowPatient). Check: Apr 1, 2026 plus 294 days is Jan 20, 2027. The US diabetes indication was still pending, per PharmExec. In the UK it was approved on Aug 10, 2026, 60 days after the Wegovy pill's Jun 11 approval (Good Fiber). If the EC confirms, Foundayo becomes the second oral GLP-1 for weight management in the EU, and unlike the Wegovy pill it also carries a type 2 diabetes label, where it meets Rybelsus.
3.3 The Wegovy pill: approved widely, launched narrowly
Novo Nordisk's Wegovy pill (oral semaglutide 25 mg) was approved by the FDA on Dec 22, 2025 and launched in the US in January 2026. Novo's Sep 30, 2026 press release says it is launched in the US, United Arab Emirates, UK and Germany, with "select markets" to follow "in the coming quarters" (company statement, no dates). The release repeats the existing Boxed Warning for possible thyroid tumors; it is not a new label change.
Europe shows the gap between approval and access. Good Fiber's tracker, a Sept 2026 snapshot, covers 16 countries:
| Wegovy pill status in 16 European countries (Sept 2026) | Countries | Source |
|---|---|---|
| On the market (UK, Germany) | 2 | Good Fiber |
| Approved, not yet launched | 13 | Good Fiber |
| Under review (Switzerland, both pills) | 1 | Good Fiber |
The three groups add up to 2 + 13 + 1 = 16. The UK has sold the pill privately since July 2026, and Germany since Sep 1, 2026, the first EU country to do so, on self-pay only. Italy's press expects a launch around October 2026, but AIFA pricing was pending (reported). The tracker is a secondary source dated before the window; it gives the Wegovy pill's EU authorisation as Jul 14, while PharmExec says only "July". It also lists Foundayo's EU status as "under review (~2027)"; the Oct 15 CHMP opinion supersedes that, and we follow the EMA.
3.4 What is still pending, and what did not happen
- CagriSema (Novo): filed with the FDA for weight management on Dec 18, 2025, with a decision expected in Q4 2026. Pharmaphorum says "December", while pepglp1 says no PDUFA date had been announced as of mid-October. We follow the quarter, not the month. Novo is targeting an early-2027 launch (Reuters, via Yahoo Finance). Status: pending.
- Retatrutide (Lilly): Lilly said on Jul 23, 2026 it plans a BLA submission in Q1 2027, according to glp3.wiki. That site's own estimate puts a first decision in late 2027 or early 2028 (estimate).
- FDA label changes: the material shows none for GLP-1 obesity drugs in the window. The nearest is the Mounjaro cardiovascular-risk indication for adults with type 2 diabetes at high cardiovascular risk, approved Aug 28, 2026, which falls before the window (according to OptiMantra).
- Other FDA actions: Rani Therapeutics' RaniPill capsule platform entered the FDA Emerging Technology Program on Oct 5, 2026. That is a programme acceptance, not a drug approval (MarketBeat). FDA's warning letter to Empower Pharmacy over compounded copies is dated Sep 18 and covered in Chapter 5.
- Generic semaglutide: India's patent lapsed on Mar 20, 2026 and copies launched the next day; China has approved zero generics (BioPharma APAC, Oct 9, 2026). Per PharmaTradz (updated Oct 19, 2026), generics are also on sale in Canada, Brazil, South Africa and Saudi Arabia (first launch Sep 16, 2026), while the US, EU, UK and Japan stay protected until about 2031–2032. Prices are in Chapter 6.
The practical reading: the next dated gates are the EC decision on Foundayo (estimate by Dec 21), CagriSema's Q4 FDA decision, and national launches of the pills already approved. Approval no longer limits oral GLP-1 access in Europe; national pricing and launch timing do.
4. Sales and market share
Chapter 3 covered which products may be sold and where; this chapter asks how much they sold and who holds the prescriptions. The short answer is that no Q3 2026 sales were reported inside the window. Lilly reports on Oct 29 and Novo on Nov 4, both after Oct 23, so every sales figure below is Q2 or H1 data.
4.1 Sales and share at a glance
| Item | Key figure | Date | Status | Source |
|---|---|---|---|---|
| Q3 results due | Lilly Oct 29, Novo Nov 4 | 2026 | Scheduled, after window | Zacks |
| Mounjaro + Zepbound sales | $27.7B, about +88%, about 65% of Lilly revenue | H1 2026 | Reported | Zacks |
| Mounjaro / Zepbound sales | $9.9B (+91%) / $4.9B (+44%) | Q2 2026 | Reported | Pharmaceutical Technology |
| Foundayo sales | $98M | Q2 2026 | Reported | Zacks |
| Wegovy + Ozempic sales | DKK 112.2B, about 75.5% of adjusted sales | H1 2026 | Reported | Zacks |
| Lilly US incretin prescription share | 60.9% | June 2026 | Reported | Zacks |
| Wegovy pill US weekly prescriptions | Over 265,000 | Week to Jul 17, 2026 | Reported | Tablets & Capsules |
4.2 Lilly's lead: sales and share
Basis matters: company figures below are reported sales, share figures are prescriptions. The material holds no Lilly or Novo earnings release, so sales sources are secondary.
On sales, tirzepatide is ahead. According to Pharmaceutical Technology, Q2 Mounjaro sales were $9.9B and Zepbound $4.9B, against $7.8B for Novo's injectable semaglutide franchise. The two Lilly brands total 9.9 + 4.9 = $14.8B, about 1.9 times Novo's figure (14.8 ÷ 7.8). Pluang puts the Q2 tirzepatide total at $14.9B, a rounding-level difference. Zacks counts $27.7B for H1, about 65% of Lilly revenue. That implies H1 revenue near 27.7 ÷ 0.65 = $42.6B; doubled, $85.2B sits at the low end of the $85–87B 2026 outlook. The Novo side: Wegovy and Ozempic made DKK 112.2B in H1, so adjusted sales were about 112.2 ÷ 0.755 = DKK 148.6B. Novo's 2026 outlook, even after being raised, is adjusted sales and operating profit between flat and −6% at constant exchange rates (company guidance, via Zacks).
On prescriptions, Lilly said in August that it held about 61% of US GLP-1 prescriptions in Q2 against roughly 39% for Novo (company statement, via CNBC). The Zacks figure of 60.9% as of June matches. Zepbound took nearly 60% of branded US obesity prescriptions by the end of Q2, and Lilly's share of international sales was 54.9% in April–May. Lilly CEO Dave Ricks told CNBC that about 700,000 seniors started a GLP-1 once Medicare coverage began in July, and 70% of them are on Lilly drugs (CEO statement, reported by CNBC around Sept 25, unaudited). Coverage terms are in Chapter 6.
Lilly leads on both sales and prescriptions: about 61% against 39% in the US, and about 1.9 times Novo's injectable semaglutide sales in Q2. The next test is Q3, reported on Oct 29 and Nov 4, after this window closed.
4.3 Novo's answer and the pill race
Novo used its Capital Markets Day, held on the Monday before CNBC's Sept 25 report (about Sept 21, inferred), to promise more than five potential multi-blockbuster launches by 2030 and more than DKK 150B (about $23B) in pipeline sales by 2035. CNBC reports that investors sold the stock, unconvinced by targets matching industry averages and by the lack of a near-term plan.
The pill is Novo's best near-term asset. Novo said in June that US Wegovy pill prescriptions passed 3 million in just over five months, with over 80% going to people new to GLP-1 therapy (company statement, secondary summaries). Weekly prescriptions then reached more than 265,000 in the week ending Jul 17, with more than 5 million cumulative. A study published Oct 13 in Annals of Internal Medicine used IQVIA National Prescription Audit data through June 2026 (peer-reviewed). It found over 100,000 new Wegovy pill prescriptions by the second month and an early uptake slope about 4.4 and 7.4 times steeper than Zepbound and injectable Wegovy. Injectable Wegovy did not fall, and the pill drove most of the net increase. The authors warn this may not last.
Foundayo is behind but gaining. Q2 sales were $98M. Pharmaceutical Technology puts oral Wegovy at about $850M for H1 and Foundayo just under $100M, all in Q2. The periods differ, so 850 ÷ 98 ≈ 8.7 overstates the gap. Ricks said one-third of new pill patients choose Foundayo and its share rises "week by week" (CEO statement, via CNBC). Leerink's David Risinger expects its share to keep growing (analyst view). Novo CEO Mike Doustdar says that, if uptake continues, pills could outnumber injectables by the end of the decade (CEO forecast).
4.4 How big the market is, and who uses it
Market-size figures disagree because they measure different things. For 2025 we have $79B (Morgan Stanley, April 2026), $66B for obesity medicines only at list prices (IQVIA), $58.4B (Precedence Research) and $52.95B (Towards Healthcare). Forecasts include $92B for 2026 (IQVIA), $200B by 2030 (J.P. Morgan) and $190B by 2035 (Morgan Stanley), all estimates. IQVIA also told a PBMI conference that GLP-1s for weight loss sold $55B over the past 12 months (+75%) and diabetes GLP-1s $98B (+16.5%). Together these total $153B, nearly double Morgan Stanley's $79B, and the article does not state the basis, so we do not use them as market size. Sources also disagree on route: injectables are 83% of sales (Towards Healthcare) or 93% (Grand View, via a Lexaria press release).
On users, Gallup found 11% of US adults were taking a GLP-1 for weight loss in mid-2026 (July, via Exploding Topics). EY-Parthenon, in a survey of nearly 15,000 people including 3,602 current or former users, found about 54% of semaglutide users take it for weight loss (CBS News, Oct 22, survey-based).
5. Capacity and supply
Chapter 4 measured how fast demand is growing; this chapter asks whether supply can follow, and how the unofficial supply from compounders is being policed. Status is judged against 2026-10-24.

5.1 Plant investment: large commitments, nothing new in the window
No company in the material announced a new plant or opening between Sep 24 and Oct 23. The figures below are cumulative, mostly undated commitments relayed by trade and research outlets.
Lilly. According to Tablets & Capsules, Lilly's announcements add up to more than $20 billion across five sites for injectable GLP-1 ingredients, products and devices. For oral Foundayo it lists $6 billion for an API plant in Huntsville, Alabama, $6.5 billion in Houston, $1.2 billion or more in Puerto Rico and $3 billion for an oral-medicines plant in the Netherlands, which sum to 6 + 6.5 + 1.2 + 3 = $16.7 billion or more. Lilly also stockpiled about $1.5 billion of Foundayo inventory before its April 2026 approval. A Towards Healthcare release of Oct 9 adds more than $3.5 billion for a Pennsylvania injectable site and $4.5 billion for Indiana. The sources do not say whether these overlap with the $20 billion, so we do not add them.
Novo. US Wegovy pills are made in Clayton and Durham, North Carolina, with ex-US supply from Athlone, Ireland and Denmark. The Athlone tableting expansion is $501 million in Tablets & Capsules and €432 million in the Oct 9 release. Since 501 ÷ 432 ≈ 1.16, a plausible euro-dollar rate, it is probably one project (our inference); neither source gives a date. After Novo's Capital Markets Day (week of Sept 21), BioSpace headlined a target of a 10-fold rise in oral Wegovy capacity by 2030, headline only. CNBC instead reports a tenfold rise in patients by 2030, with executives saying supply is not an issue. These are different claims, and neither has a baseline.
Market estimates. Research and Markets, in a paid press release dated Oct 19, values GLP-1 manufacturing at $45.0 billion in 2025, growing 26.0% a year to about $180 billion by 2031 (estimate). Check: 45.0 × 1.26^6 ≈ 180. It also estimates that $18.48 billion of $24.86 billion in 2025 CDMO investment went to US and non-China sites, 18.48 ÷ 24.86 ≈ 74%. Scope is undefined, so the $45.0 billion should not be compared with Towards Healthcare's $52.95 billion 2025 drug-market estimate.
Oral products change the picture. Leerink analyst David Risinger expects the small-molecule Foundayo to be cheaper to make at scale than the peptide Wegovy pill, and Sharp Services partner content says widespread oral shortages are less likely.
5.2 Compounded GLP-1s: one recall and one landmark letter
Apothecary Pharma recall. Apothecary Pharma LLC of Cary, North Carolina, began a voluntary nationwide recall on Sept 24 for "lack of assurance of sterility". It covers tirzepatide 18 mg/mL in 4 mL multidose vials (NDC 85091-001-18), lots AP-202622 to 202625 and 202627 to 202644, plus one NAD+ lot, AP-202647. The range is 4 + 18 = 22 lots, 23 with the NAD+ lot. FDA says "22 vials", but USA Today notes the codes suggest 22 lots and asked FDA to clarify. FDA classified the recall Class II on Oct 8 according to FOX and the Boston Globe, and on Oct 10 according to USA Today and the Democrat and Chronicle. We follow Oct 8. The notice reports no illnesses. FDA warned the firm in December 2025 (the Boston Globe says March; four other sources say December), but has not tied that letter to the recall. Beyond-use dates run from Oct 7 to Nov 26; four lots had passed theirs by Oct 24. Mounjaro and Zepbound are unaffected. For scale, the July 29 recall by 503B facility Apollo Care covered more than 162,000 semaglutide vials, about 7,000 times 23 (162,000 ÷ 23).
Empower warning letter. FDA's letter #738238 to Empower Pharmacy, a Houston 503A pharmacy, is dated Sept 18 and was reported on Sept 24. It followed a November 2025 inspection and three Empower responses. FDA found tirzepatide/niacinamide and semaglutide/cyanocobalamin products to be "essentially copies" and said volumes suggest the differences are "pretextual". Some orders had no prescriber determination of a significant difference, others repeated one verbatim, and FDA questioned third-party platforms that pre-select the wording. FDA gave Empower 15 working days to reply; if the letter arrived Sept 18, that clock ran to about Oct 9 (our calculation), and no reply is reported. NASPA and a law-firm note stress this is not a categorical ban on compounding either drug.
The ladder around it. FDA sent 30 warning letters to telehealth sellers on Mar 3 and about 25 more on June 8 (law-firm figure). On Apr 30 it proposed excluding semaglutide, tirzepatide and liraglutide from the 503B bulks list; according to NexLife, no final decision existed at Oct 8, and the Fifth Circuit upheld FDA's shortage decisions on Aug 27. Use of compounded drugs has fallen from about 1 in 3 GLP-1 users in 2024 (KFF) to about 1 in 5 (Gallup, July).
5.3 Supply status: the shortage list is clear
FDA declared the tirzepatide shortage over in October 2024 and reaffirmed it on Dec 19, 2024; SmithRx gives October 2024, and we use Dec 19 as the final determination. The semaglutide shortage ended Feb 21, 2025. As of April 2026 neither drug appears on FDA's list, though "localized supply disruptions can still occur". No check dated inside the window was found. Grace periods for copies ended Mar 5 and Mar 19, 2025 for tirzepatide (503A, 503B) and Apr 24 and May 22, 2025 for semaglutide. One blog gives Apr 22 for 503A semaglutide; we follow Apr 24 from two law-firm sources.
In this window, supply news was enforcement, not scarcity: a 23-product recall and one warning letter, while the plant figures are cumulative commitments with no in-window announcement.
With no shortage, compounding rests on the individualized-prescription argument, which the Empower letter attacks directly. Every figure in this chapter is secondary, and most plant figures are undated.
6. Coverage and pricing
Chapter 5 covered how much drug can be made; this chapter covers who pays for it and how much.
| Item | Key figure | Date | Status | Source |
|---|---|---|---|---|
| Medicare GLP-1 Bridge | $50 copay; net price $245 | Jul 1, 2026 – Dec 31, 2027 | In effect | MyClearMatch |
| Medicaid obesity coverage | About 13 state programs | Jan 2026 | Reported | YourHealthier |
| TrumpRx.gov cash target | ~$350 toward ~$245 | Launched Feb 5, 2026 | Target (estimate) | JumpstartMD |
| Wegovy pen / Zepbound, cash | $349 / $399 | Oct 19, 2026 | In effect | GLP Chart |
| Zepbound commercial coverage | 60% uncovered | Jul 2026 | Reported | GoodRx |
| Employer plans | ≥10% may drop coverage in 2027 | 2026 survey | Reported | Vox |
The US price of a GLP-1 now depends on the channel. Medicare pays a $50 copay and cash buyers pay $149–$449, while commercial insurance, the largest payer group, is shrinking.
6.1 Federal programmes: Medicare Bridge, Medicaid, TrumpRx
The CMS GLP-1 Bridge, a demonstration outside Part D, has been in effect since July 1, 2026 and ends December 31, 2027. Eligible beneficiaries pay a flat $50 per monthly supply; according to MyClearMatch, the government negotiated a $245 net price with Novo Nordisk and Eli Lilly. Covered are Wegovy (injection and tablet), the Zepbound KwikPen and Foundayo, not Ozempic or Mounjaro. Eligibility is tiered by BMI at the start of therapy: 35 or higher; 30 or higher plus a listed condition such as uncontrolled hypertension; or 27 or higher plus prediabetes or a prior heart attack or stroke. The copay does not count toward the $2,100 Part D out-of-pocket cap.
At the PBMI conference on Sept 10–11, IQVIA's Scott Biggs said the first three days showed 9,000 paid claims, "almost $2 million" from CMS. Cross-check (our arithmetic): 9,000 × ($245 − $50) = $1.76 million, which fits; at the full $245 it would be $2.2 million.
Only about 13 state Medicaid fee-for-service programs covered obesity GLP-1s as of January 2026; JumpstartMD says this was down from 16.
TrumpRx.gov launched Feb 5, 2026. Cash prices are expected to start near $350 and fall toward $245 over about two years, a target and not a current price. EY-Parthenon, via CBS, says the administration's agreement with the two makers cut prices to $150–$350 a month from roughly $1,000. No official TrumpRx or CMS page is in the material.
6.2 Cash prices: what was posted on Oct 19
GLP Chart, which checks sellers' pages weekly, gives the newest same-source set.
| Product and channel (checked Oct 19, 2026) | Monthly cost | Note |
|---|---|---|
| Wegovy pen, NovoCare | $349 | Flat across doses; cut from $499 in Nov 2025 |
| Wegovy pill, NovoCare | $149 / $299 | $149 for 1.5 and 4 mg; $299 for 9 and 25 mg |
| Wegovy pill, Novo 12-month plan | $249 | Telehealth partners only; $323–$398 with fees |
| Zepbound, LillyDirect | $399 | 5 mg; from $299 at starter dose |
| Zepbound KwikPen, Sam's Club | $399–$449 | $449 at higher doses, refills inside 45 days |
List prices were about $1,349 for Wegovy and $1,086 for Zepbound in June 2026; YourHealthier gives $1,350 and $1,060. Cash saves a lot: 12 × $349 = $4,188 a year for the Wegovy pen, against 12 × $1,349 = $16,188 at list, about 74% less. A commercial copay after prior authorization is about $25, but savings cards cannot be used under Medicare, Medicaid, VA or TRICARE.
Some offers have lapsed or are unchecked. NovoCare's $199 starter offer ended June 30, 2026. June LillyDirect vial prices ($299, $399, $449) were not re-checked in October. Compounded semaglutide is advertised from $76 all-in, but is not FDA-approved.
Outside the US the material is thin. The Wegovy pill has sold in Germany since Sept 1, 2026 for about €170–€280 a month, self-pay only, as statutory insurance does not cover it. UK sales of the Wegovy pill (since July) and Foundayo (since August) are private, not NHS, and the material gives no UK price. Health Canada approved the first generic semaglutide on Apr 28, 2026; Canadian generics typically sell 45–90% below brand.
6.3 Commercial insurers and employers: coverage is retreating
GoodRx's July 2026 tracking shows Zepbound uncovered for 60% of commercial lives, up from 51% in 2025 (over 114 million people, 17 million more), with restricted coverage down from 45% to 37%. For Wegovy, 27 million are uncovered; over 66 million lack Foundayo cover; 88% of covered patients face prior authorization or similar rules. The figures are consistent: 114M ÷ 0.60 ≈ 190M commercial lives. GoodRx reports Cigna stopped covering Wegovy and Zepbound for employees (date not stated) and CVS Caremark returns Zepbound to its standard formulary in October 2026, day not given, so we cannot confirm it is in effect. A GEHA plan document shows the pattern: from Jan 1, 2026 Wegovy was non-preferred and Zepbound non-formulary, needing an exception.
Vox, undated, reports about two-thirds of private employer plans covered weight-loss GLP-1s in 2026. A Business Group on Health survey found at least 10% of covering plans likely to stop in 2027; Mercer found 6% of large employers dropped coverage in 2026 and 5% more plan to or are considering it; Massachusetts' Health Policy Commission found commercially insured GLP-1 users fell 50% from 2025 to Q1 2026; PepsiCo, Starbucks and Lowe's are reportedly cutting. Nearly 8 in 10 employers say GLP-1s raise costs. Canada runs the other way: employers covering weight-loss GLP-1s rose from 17% in 2024 to 37% in 2026.
The market is split. A Medicare patient or cash buyer has a stable, published price; a commercially insured patient faces a plan decision moving against coverage.
7. Outlook and dates
Chapters 2 to 6 covered the trials, the decisions, the sales, the supply and the prices. This chapter turns them into a reader-by-reader table and a dated watch-list. It uses only facts from those chapters and gives no medical or investment advice.
7.1 What the month changed
Three things changed. First, the oral pills moved from approval to access: the CHMP backed Foundayo on Oct 15, the Wegovy pill is launched in four markets, and only 2 of 16 European countries had it on sale in September. Second, the evidence base widened without a record: a heart-safety result for an oral drug, and the first randomized data on limiting regain, such as Fractyl's 5.1% against 8.2%. Third, payment fragmented: Medicare and cash prices are published, while commercial coverage shrinks.
Access and price, not approval or efficacy, now decide who gets a GLP-1: Lilly leads with about 61% of US prescriptions, the pills are approved faster than they launch, and coverage depends on the channel.
The next test is financial. Lilly's Q3 results on Oct 29 and Novo's on Nov 4 are the first sales data after the window, and both companies' H1 figures are the baseline.
7.2 What it means by reader
| Reader | What changed | Implication | Evidence |
|---|---|---|---|
| Patients in the US | Medicare Bridge has run since Jul 1 with a $50 copay; cash prices are $349 for the Wegovy pen and from $149 for the Wegovy pill | The route to a GLP-1 depends on the payer: a Medicare patient and a cash buyer have a published price, a commercially insured patient faces a plan decision | — |
| Patients in Europe | Foundayo has a CHMP opinion; the Wegovy pill is approved in 15 of 16 tracked countries but on sale in 2 | Access depends on national launch and pricing, and Germany's pill is self-pay at about EUR 170-280 a month | — |
| Payers and employers | Zepbound is uncovered for 60% of commercial lives; at least 10% of covering plans may stop in 2027 | Coverage is a cost decision under pressure; the Bridge ends Dec 31, 2027 | — |
| Investors | Lilly leads Q2 sales about 1.9 times and prescriptions about 61% to 39%; Novo guides adjusted sales flat to -6% | Q3 results on Oct 29 and Nov 4 will test whether the lead widens; the Foundayo ramp ($98M in Q2) is the number to watch | — |
| Compounders and telehealth sellers | FDA's Empower letter calls products "essentially copies"; a sterility recall is ongoing; no shortage is listed | Compounding rests on the individualized-prescription argument, which FDA is attacking | — |
