01Overview

The quarter moved on four fronts, but no new gene editor or CAR-T product was approved

Between Jul 25 and Oct 23, 2026, approvals, clinical data, payment design and dealmaking each shifted. Pick a takeaway, then filter the nine therapies that carried the quarter.

Five takeaways, each judged against Oct 24, 2026

Approvals went to AAV and oncolytic products; the first in vivo CRISPR decision is still ahead

Citeline counts 49 approved gene therapies at Q3, up from 46 at Q2, which equals three US approvals: Fayuvi (standard full approval, Sept 17), Genglycos and Tudriqev (accelerated approval, Aug 6). Casgevy was extended to children aged 2 and older in July, and Vertex reported Casgevy revenue of $76.4M for Q2 2026, up 151% from $30.4M. Lonvo-z has Priority Review with a goal date of Mar 10, 2027 (filed).

In vivo liver editing gave the clearest numbers; autoimmune CAR-T gave the strongest signal and the first fatal safety event

BEAM-302 at 60 mg lifted total AAT from 5.0 to 14.4 µM, above the 11 µM threshold, in a company-reported Phase 1/2 analysis of 29 patients. Lonvo-z cut HAE attacks to 0.26 versus 2.10 per month in Phase 3 (reported −87%). In autoimmune disease, 22 of 24 patients were drug-free at 24 weeks in Erlangen's basket trial, but Novartis paused its rap-cel trials on Aug 24 after 3 deaths; no lift was reported by Oct 23.

Medicaid buys sickle cell gene therapy through one CMS framework, but net prices remain unpublished

The CMS model covers 32 states, DC and Puerto Rico, representing 84% of Medicaid beneficiaries with the disease (in effect), while list prices are $2.2M for Casgevy and $3.1M for Lyfgenia. At end-2025, 64 Casgevy infusions and 115 Lyfgenia patients treated were reported, against an estimated 25,000–35,000 eligible US patients. Employers carry the exposure: million-dollar-plus claims rose 46% from 2022 to 2026 (reported via CaptivePFTN), and 2027 cost-trend surveys (estimates) run 8.2% to 11.1%.

Capital moved to in vivo platforms on staged terms, while ex vivo assets were sold, returned or wound down

J&J's option on Sail Biomedicines pays $785M up front (22.4% of $3,505M potential, with a $2.58B option payment) for a preclinical lead. PTC won Sangamo's ST-920 at auction for $111M upfront, BMS ended a $380M Cellares agreement with 100 layoffs, and Citeline reports Q3 deal volume down 33%.

Delivery is proven for the liver and not elsewhere, and long-term follow-up is the thinnest evidence

Nature called Prime Medicine's PM577a the first known prime editing in a person, with one patient dosed and data due in 2027. Beyond the liver, evidence is animal, in vivo CAR-T has no mature human data, and a scoping review found 29 genome-editing trials, only 6 of them in vivo. Reported umbrella-trial designs would let variants be added under one IND, with a formal trial expected to enrol in 2027 (reported).

Pathways, Europe and what is pending

FDA guidance Pre-window

Draft guidance on genome-editing products (Feb 2026) envisions adapting an approved editor to related mutations given a plausible mechanism. Draft guidance on “Leveraging Prior Knowledge” followed on Jun 2, 2026, and final CMC flexibilities guidance on May 5, 2026 ended the “rule of three” batches.

In the window Reported

Industry comments urged the FDA to widen the prior-knowledge guidance and link it to the plausible-mechanism framework and platform designation (AgencyIQ; date not stated). No platform designation for a gene-editing product appears. Kiran Musunuru said on a podcast that the FDA cleared the Baby KJ IND in one week and has agreed to umbrella designs, with a formal trial expected to enrol in 2027.

BEAM-302, Sept 8, 2026 Company-reported

Lipid-nanoparticle adenine base editor for AATD, presented at the European Respiratory Society Congress (open-label Phase 1/2). By Aug 17, 38 patients were dosed; the analysis covers 29 (June 24 cutoff). Infusion reactions affected 41%; one Part B patient had transient Grade 3 ALT/AST elevations.

Path to approval (company)

Beam seeks accelerated approval on a 12-month AAT biomarker endpoint, enrolling about 50 more patients; the first pivotal patient was dosed in July. China's YolTech reported YOLT-202 in 4 patients, lifting AAT from about 5 to 20 µM through six months (via CRISPR Medicine News; date not stated).

VERVE-102 Published (NEJM)

NCT06164730, funded by Verve. PCSK9 fell 51% to 88% and LDL 9% to 62% across six doses; 15 of 35 participants had at least one year of follow-up. Adverse events were mild-to-moderate infusion reactions, transient ALT elevations and one aspiration pneumonitis.

CTX310 Phase 1a data

Triglycerides fell 48% at 0.8 mg/kg (LDL −53%); Phase 1b data are expected in H2 2026. Both cardiovascular datasets are small and measure blood markers, not cardiac events. CTX340 (refractory hypertension) and CTX460 (AATD) started Phase 1 without data.

PM577a Company-reported

Cleared by Medsafe (June) and the FDA (July); the first patient was dosed in October (the day is not confirmed), with data in 2027. For PM359 in chronic granulomatous disease, the company cites positive proof of concept in its first two patients and targets a BLA in the first half of 2027 (company plan).

Beam pipeline Upcoming

BEAM-301 first-in-human data are expected in 2026 and an updated risto-cel dataset by year-end (company guidance).

Efficacy signal Published

Erlangen's basket trial reported drug-free control in 22 of 24 patients at 24 weeks, with no ICANS or severe CRS (Nature Medicine). Kyverna's miv-cel won RMAT in August for non-active secondary progressive MS on six Stanford patients; no disability or safety data were released, and more is due in Q4 2026. A 2025 pooled analysis of 145 patients (context only) found remission in 70% of lupus patients.

In vivo CAR-T No mature data

A Sept 3 NEJM correspondence reported lentiviral in vivo CD19 CAR-T activity in neurologic autoimmune disease; one source puts it at 16 patients without a control group. Tempest and Hebei Senlang plan about 10 patients, first dosing Q4 2026. AbelZeta's C-CAR168 won FDA clearance for a registrational Phase 2 in refractory lupus nephritis in July.

Safety break, Aug 24, 2026
3 deaths

Novartis paused rap-cel autoimmune trials after 3 deaths from immune effector cell-associated hemophagocytic syndrome. Bristol Myers Squibb paused zola-cel enrolment. Neither pause was reported lifted by Oct 23.

Sources differ Reported

Rare Disease Advisor says BMS called the events transient and reversible; Nature Medicine says BMS cited more ICANS in a Phase 2 trial. BMS's own statement is not in the material, so the two cannot be reconciled.

The pauses raise the bar for autoimmune CAR-T filings, and in vivo CAR-T remains a design bet, not an evidence base.
04Pricing and payment

Medicaid buys sickle cell gene therapy through one CMS framework, but every price shown is a list price and net prices stay unpublished

Payment design, not sticker price alone, decides access. No net price appears in the material; CMS says only that negotiated terms include pricing discounts and outcomes-based rebates.

Casgevy revenue in context Estimate

$76.4M in Q2 2026 is about 35 courses' worth at the $2.2M list price, an order of magnitude only, since revenue is global.

Who pays, and what happens when economics fail

Cell and Gene Therapy Access Model In effect (CMS)

A voluntary, multi-year CMS model in which CMS negotiates key terms with manufacturers, then states decide whether to sign. Vertex and Genetix each signed a separate agreement. Under an outcomes-based agreement, a manufacturer refunds part of the price if the therapy does less than expected, and may add guaranteed rebates.

84%of Medicaid beneficiaries with the disease: 32 states, DC and Puerto Rico
11performance years, ending Dec 31, 2035; evaluation reports from 2027

Medicaid covers between 50% and 60% of people with the disease. Start was rolling, from the first state on Mar 1, 2025. CMS counts 34 programs; Mordor Intelligence says 35, and we follow CMS.

Virginia Anticipated

DMAS told stakeholders in August 2026 it would join from Oct 1, 2026, carve the drug out of inpatient bundles and cover follow-up for up to one year. The date has passed and the material does not confirm it happened.

Not disclosed

Rebate percentages, outcome thresholds, measurement windows and net prices. Mordor, citing CBO, says sickle cell therapies could reach $1.8B a year in Medicaid spending by 2028 (estimate; the CBO document is not in the material).

Million-dollar claims Reported

CaptivePFTN, a captive-insurance adviser with a commercial interest, wrote on Aug 10, 2026 that Sun Life's 2026 report (70,000+ high-dollar claims, 3,300+ self-funded employers) shows million-dollar-plus claims up 46% in frequency from 2022 to 2026, with Elevidys averaging $3.6M per claim. UnitedHealthcare (Aug 6, 2026) says claims of $100,000 or more rose nearly 13% in 2025.

Stop-loss illustration Our arithmetic

Above a $500,000 attachment, a Casgevy-priced claim leaves the carrier $2.2M − $0.5M = $1.7M, or 77%; a Lyfgenia-priced claim leaves $3.1M − $0.5M = $2.6M, or 84%. Not a quoted case.

Marsh: 8.2% after planned changes, 11% without. Aon: above $19,000 per employee. CaptivePFTN lists carve-outs, centers of excellence, installment and “no cure, no pay” contracts as plan levers.
End-2025 uptake (company reports via CNBC, Oct 16, 2026)
64Casgevy infusions worldwide; 147 first cell collections (Vertex)
115Lyfgenia patients treated in the US; 216 collections initiated (Genetix)
25,000–35,000eligible US patients, 25%–35% of about 100,000 (estimate)
< 1%179 infused (64 + 115) as a share of that pool; an upper bound because Vertex's count is global

Collections outnumber infusions by about 2.3 (147 ÷ 64) and 1.9 (216 ÷ 115), a backlog because treatment can take a year. Barriers are operational: conditioning chemotherapy that often causes infertility, weeks in hospital, apheresis scheduling, REMS compliance and follow-up of up to 15 years.

The failure case: hemophilia A Reported

BioMarin withdrew Roctavian after only $36M of 2025 sales, taking a charge of about $240M; Pfizer's Beqvez was also withdrawn in 2025 (DrugPatentWatch, Sep 18, 2026). A peer-reviewed paper dated July 28, 2026 calls the Roctavian exit a failure of economic sustainability, market access and system readiness, not safety or efficacy.

Sickle cell now has a federal outcomes-based template, but employers and health systems still carry the risk and coordination burden. Without published net prices and rebate terms, outsiders cannot judge whether it lowers cost.

What buyers paid for the next generation, and which assets were sold, returned or shut down. Potential values are headline values, not cash paid.

J&J option on Sail Biomedicines ($M)

Total potential $3,505M = 785 + 140 + 2,580. All three components come from the J&J release (Jul 29, 2026); the $785M includes $465M equity, and the other $320M is not itemised.

A staged bet on in vivo CAR-T

Announced Jul 29, 2026

J&J paid 22% of Sail's potential value up front and kept the rest as an option. The deal is subject to regulatory approvals and other conditions; the option was not exercised in the material. J&J expects exercise to dilute adjusted EPS by about $0.18 in 2026 and $1.28 in 2027.

Lead asset: sources disagree Preclinical

J&J names none. CGTlive and pharmaphorum call it SAIL-0839 (CD4 and CD8 cells, autoimmune); AllSci calls it SAIL-0804 (anti-CD19). We follow SAIL-0839. Either way there is no human data.

Context: Lilly-Kelonia Estimate

CGTlive reports $3.25B upfront; others say up to $7 billion. If read as an upfront inside a larger headline, about 46% (3.25 ÷ 7) was paid up front against Sail's 22%.

Sangamo, Aug 12, 2026

PTC won the auction for ST-920 with $111M upfront plus up to $100M on regulatory approvals, a maximum of 111 + 100 = $211M. According to BioBucks (reported), the auction cleared at $163.55M cash, beating stalking horse Astellas, and Lilly paid $50M for the capsid, zinc finger and MINT platforms and ST-506. PTC plus Lilly is $161M, so $2.55M is not itemised.

Retreats Reported

Cellares is cutting 100 jobs after BMS ended its contract, with terminations due by Oct 20. A BMS spokesperson said the Cell Shuttle could not meet commercial-scale needs for Breyanzi; Cellares disagrees. Delivery, in vivo CAR-T and personalised editing

Prime editing reached the body Company-reported

Nature called PM577a “the first known instance” of prime editing in a person (Oct 22); no in vivo editing therapy is approved yet. The first patient was treated “earlier this month” after a New Zealand CTA (June 2026) and FDA IND (July 2026); the trial starts in stable adults and tests safety, dose and copper excretion. H1069Q accounts for about 30–50% of ATP7B mutations in the US and Europe (company, via Chosun Biz).

One platform, several programmes

The same universal liver LNP underpins PM647 for AATD; CRISPR Medicine News (Sep 25) reports FDA clearance of its Phase 1/2. CRISPR Therapeutics runs CTX340 and CTX460 on its own liver LNP (Phase 1 by Aug 3, 2026). VERVE-102 uses a GalNAc-bearing LNP. Chemistry still improves, but in animals: fully modified pegRNAs raised mouse liver editing to 67% (preclinical).

Evidence is animal Preclinical

A Huntington's base editor was delivered by split-intein AAV to rodent striatum.

Open problems named by reviews

Endosomal escape, extrahepatic targeting and the “PEG dilemma” (Jun 8, 2026); delivery is called the core bottleneck (ACS Nano, Oct 6, 2026).

Lentiviral Reported

IASO Biotechnology's CD20 candidate IASO208 reportedly received an FDA IND in the second half of September, per a Japanese industry newsletter.

mRNA/LNP Announced

Tempest's TPST-4003 pairs CD7-targeted delivery with a CD19/BCMA CAR; first dosing expected in Q4 2026 (Tempest release, Aug 13). The newsletter says Tempest holds a lentiviral candidate in Phase 1; we follow Tempest.

Circular RNA Preclinical

Sail combines eRNA with targeted LNP; its lead is preclinical. RiboX's RXIM002 reportedly cleared a US IND and CREATE's CRT-402 Australian ethics approval (newsletter, first-half August).

The only human in vivo CAR-T result here is a Sept 3, 2026 NEJM correspondence on lentiviral CD19 CAR-T in MS and NMOSD. A review (Immunity & Inflammation, Aug 31, 2026) says both LNP-mRNA and targeted lentiviral systems transduce T cells efficiently in primates and flags short mRNA persistence and repeat-dosing immunogenicity. Nature says in vivo products “might be much cheaper and faster”, with no cost data.

Bespoke and umbrella designs Reported

Per Kiran Musunuru on a podcast (date not stated), the FDA cleared the IND for the 2025 neonatal CPS1 base-editing therapy in one week, design to infusion took six months, and the FDA has agreed to umbrella designs, with a formal trial expected to enrol in 2027. That is a researcher's account, not an FDA document. At ASGCT 2026 he proposed adding a variant on cellular on- and off-target data alone (a proposal, not an accepted rule); cGMP and process validation are still required.

Off-target and follow-up

Tracking-seq2 found that genomic variation drives off-target differences between individuals. The scoping review found 29 trials: 23 used CRISPR-Cas9 (79%), all were Phase I/II and 6 (21%) were in vivo. A UCL workflow reached 61% knock-in in more than 100 million CD34+ cells, but the corrected fraction fell after transplantation in mice.

Judgement: delivery is proven for the liver and not elsewhere. First 2027 data from PM577a and TPST-4003 will show whether it generalises; follow-up beyond one year is the gap.

What changed for each reader, what is unresolved and what is due next. It uses only facts from the modules above and is not medical or investment advice.

What changed, by reader

What changed

Casgevy extended to ages 2+ (about 5,500 newly eligible, company estimate); Fayuvi approved Sept 17 with shipment to Qualified Treatment Centers expected within 30–60 days.

Implication

Capacity, not eligibility, is the limit: 179 patients infused by end-2025 is under 1% of an estimated 25,000–35,000 eligible US patients.

What changed

FDA approvals went to AAV and oncolytic products; accelerated approval for Tudriqev rests on IGNYTE-3 data expected in 2030; autoimmune CAR-T trials were paused after 3 deaths.

Implication

Confirmatory evidence and safety follow-up now decide timelines; in vivo CAR-T has dosing plans but no mature human data.

What changed

One CMS outcomes-based model covers 32 states, DC and Puerto Rico (84% of Medicaid beneficiaries with sickle cell disease, in effect); million-dollar claims up 46% in self-funded plans (reported).

Implication

Medicaid has a template but net prices are unpublished; employers still carry the risk, and a $500,000 stop-loss leaves a Casgevy-priced claim 77% with the carrier (our arithmetic).

What changed

J&J paid $785M of $3,505M potential value up front for a preclinical in vivo CAR-T lead (announced Jul 29); Q3 deal volume fell 33% (Citeline).

Implication

Staged terms favour in vivo platforms; ex vivo assets without that story were returned or wound down.

What changed

Prime editing reached a person (company-reported); the FDA drafted guidance on prior knowledge (Jun 2) and umbrella designs are reported.

Implication

Liver delivery is the proven route; broader platform rules are still drafts.

Unresolved on Oct 24, 2026

The rap-cel and zola-cel pauses were not reported lifted by Oct 23. PTC's closing of the ST-920 purchase was expected in late Q3 or early Q4 2026 and is not confirmed. Virginia's Medicaid start was anticipated for Oct 1 and is not confirmed. No exercise decision date is stated for J&J's Sail option.

Evidence gaps that limit any forecast

No net price or rebate term is published for the CMS model or any therapy. There is no Casgevy patient count after the end-2025 figure of 64 infusions, and nothing on Zolgensma or Hemgenix launches. The scoping review found 29 genome-editing trials, all Phase I/II, with later-timepoint outcomes reported less often than 3- and 6-month results.
